This Website Privacy Policy explains how HarmonyVille Behavioral Health (“HarmonyVille,” “we,” “us,” or “our”) collects, uses, and protects information when you visit www.harmonyvillepsych.com, contact us, request an appointment, or communicate with us online, by phone, by text message, or by email. It applies to information collected through our website and related online tools. It is separate from our Notice of Privacy Practices, which explains how we handle your protected health information (PHI) under HIPAA and is included in this document and available on our website and at each office.

1. Information We Collect

Information you give us. When you use an online form, call, text, or email us, you may provide your name, your child’s name, date of birth, phone number, email address, preferred location, insurance information, the reason for your request, and any other details you choose to share.

Information collected automatically. When you visit our website, we and our service providers may automatically collect technical information such as your IP address, browser and device type, pages viewed, time on the site, referring page, and approximate location. This is typically collected using cookies and similar technologies.

Please do not use our website forms for emergencies or to send detailed medical or mental health information. Share only what is needed to request an appointment or ask a general question. If you or someone else is in crisis or in immediate danger, call 911 or dial 988 (Suicide & Crisis Lifeline).

2. How We Use Information

  • To respond to your questions and requests, schedule and confirm appointments, and verify insurance.
  • To provide treatment, process payment, and operate our practice, as described in our Notice of Privacy Practices.
  • To send appointment reminders, follow-ups, and administrative messages.
  • To operate, secure, and improve our website and understand how it is used.
  • To measure the performance of our advertising, using the limited technologies described in Section 4.
  • To comply with legal obligations and protect the rights and safety of our patients, staff, and practice.

3. How Online Requests Are Handled

Information you submit through our online forms is received into our secured Microsoft 365 environment, which we maintain in a manner designed to meet HIPAA requirements, including a Business Associate Agreement with the provider. Only authorized staff who need the information to respond to you can access. Once you become a patient, information you provide is handled as protected health information and is recorded in our electronic health record. Internal scheduling and tracking logs are limited to the minimum information necessary, and sensitive identifiers are redacted.

4. Cookies, Analytics, and Advertising

We use cookies and similar technologies to make our website work, to understand website traffic, and to measure our advertising. We use Google Ads and related Google measurement tools to show our practice to people searching for care and to measure whether our ads lead to contact requests.

We do not knowingly allow advertising or analytics tools to receive information about your health condition, your appointments, your treatment, or any information that identifies you as a patient. We do not place advertising or tracking technologies on password-protected patient portal pages. We do not sell your personal information, and we do not use patient health information for advertising.

You can control cookies through your browser settings and can opt out of Google personalized advertising at adssettings.google.com. Blocking some cookies may affect how parts of the website work.

5. Text Messages and Email

If you give us your mobile number and agree to receive texts, we may send appointment reminders, confirmations, and administrative messages. Message and data rates may apply, and message frequency varies. You can opt out at any time by replying STOP, and ask for help by replying HELP. Mobile phone numbers and text-messaging consent are not shared with third parties or affiliates for their marketing purposes.

Text and standard email are not always secure. Please do not send sensitive health details by text or regular email. By giving us your number or email address you confirm we may use it to contact you, and you may ask us at any time to use a different method.

6. How We Share Information

We share information only as described here and in our Notice of Privacy Practices:

  • Service providers and business associates that help us run our practice (for example, our electronic health record, scheduling, telephone, messaging, billing, cloud, and website vendors). They must protect the information and use it only to perform services for us.
  • Your care and payment: with other health care providers and your insurer, as permitted by law.
  • Legal and safety reasons: when required by law, court order, or to prevent serious harm.
  • With your written permission. We do not use or share patient photographs, videos, testimonials, or other identifying materials for any purpose other than care without a signed authorization, which you may revoke at any time.

We do not sell personal information.

7. Third-Party Links and Tools

Our website may link to or embed third-party tools, such as online scheduling platforms, our patient portal, payment systems, maps, and social media. These operate under their own privacy policies, and we do not control them. Please review their policies before sharing information.

8. Children’s Privacy

Our website is intended for parents, guardians, and adult patients. We do not knowingly collect personal information online directly from children under 13. We provide care to children, adolescents, and adults. Parents or guardians provide information on behalf of minor patients. If you believe a child has submitted information without a parent’s or guardian’s permission, contact us and we will work to delete it.

9. Security

We use administrative, technical, and physical safeguards designed to protect information, including access controls, encryption where appropriate, workforce training, and vendor agreements. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. If a breach of your information occurs, we will notify you and government authorities as required by law.

10. How Long We Keep Information

We keep patient records for as long as you remain a patient of our practice and for any longer period required by federal or state law. Records are not deleted solely because a patient has not visited recently; they remain in our system until the patient has been transferred to another provider or location, or as otherwise permitted or required by law, after which they are retained and securely disposed of in line with legal requirements. Website inquiries from people who do not become patients are kept only as long as reasonably needed to respond and for our business records.

11. Your Choices and Rights

You can ask us to correct your contact information, stop non-essential messages, or use a different way of contacting you. Your rights over your medical records, including access, amendment, and an accounting of disclosures, are described in our Notice of Privacy Practices. To make a request, contact our Privacy Officer below.

Texas residents. Texas law, including the Texas Medical Records Privacy Act (Chapter 181 of the Texas Health and Safety Code), provides additional protections for medical records and health information, including limits on how records may be disclosed, required privacy training for our workforce, and a requirement that we notify you that your information may be subject to electronic disclosure. We comply with these laws in addition to HIPAA.

12. Changes to This Policy

We may update this policy from time to time. The current version will always be posted on our website with its effective date. Material changes affecting how we handle protected health information will also be reflected in our Notice of Privacy Practices.
